Our Mission

All names Case Managers at Mental Capacity Consult (MCC) Case Management are regulated professionals such us Nurses, Physiotherapists, Psychologists, Occupational Therapist, Speech and Language Therapist, and Social Workers. We combine our valuable diversified skills and knowledge to home in on the holistic needs of our individual clients.
Our Case managers manage the cases of clients who have suffered personal injuries such as brain damages, spinal cord injuries among others who otherwise would not be able to coordinate their affairs following their injuries.
Our Case Manager’s responsibilities include coordinating the services needed to address our clients’ needs for social, health, educational, and employment as well as family assistance. This entails closely coordinating with the larger and diversified team of professionals in our team and external services and advocating for the individuals. Our Case Manager have strong communication skills, and we encourage collaboration to make use of all available resources to achieve high-quality, reasonably priced outcomes.
When our clients and the Case Manager first identify their needs, we offer suggestions on how to address these needs and what is required to meet them. Following that, the Client and Case Manager would collaborate to develop and carry out plans that might entail hiring a multidisciplinary team, providing a support worker package, or encouraging the client to participate in more community events. Regular reviews and progress reports to the client, family, and participating professionals will be provided by our Case Managers.
Some of the main responsibilities of our case managers are:
- Accepting referrals and keeping the referrer updated regarding the work with the client.
- Person centred approach at all times.
- Develop a therapeutic working relationship with the client and family
- Maintain accurate chronological clinical records of all work undertaken with the client.
- Use current best practice and research to guide intervention.
- Work continually to achieve optimum clinical outcomes for the client in as cost effective a way as possible.
- Identify and commission the most appropriate services to meet the client’s identified clinical needs
